Directions: To place a referral (Hospitals or providers only)
Step 1: Enroll in the LeadingReach Network
Before a provider can send a referral, they must enroll in the LeadingReach Network.
Visit the LeadingReach website and find the enrollment or "Join the Network" page.
Complete the free enrollment form by providing the requested information.
Step 2: Add the patient to the system
Next, the provider must log in to their new LeadingReach account and add the patient.
Navigate to the "Patients" tab or locate the "Add a New Patient" function.
Enter the patient's required information.
Step 3: Create and send the referral
Once the patient is in the system, the referral can be created and sent.
Click the "Send" button in the menu.
Select the patient who needs the referral.
Search the LeadingReach Network for the specialist or organization you are referring to.
Attach all necessary documents, such as patient demographics, insurance information, lab results, and recent progress notes. Files must be in PDF format.
Provide a clear reason for the referral, including any diagnosis and ICD-10 codes.
Add any optional notes or additional information for the receiving provider.
Review the referral details and send.
Step 4: Track the referral
LeadingReach offers features to track the referral's status, providing real-time visibility into care transitions and confirmation of patient appointments. The system reduces reliance on faxes and phone calls and provides a secure, HIPAA-compliant way to communicate with other offices.
